Care Coordinator/Case Manager
We’re seeking a caring, detail-oriented Care Coordinator/Case Manager to support individuals transitioning from incarceration. This non-clinical role provides person-centered care coordination and case management, including personal assistance, mental health coaching, and education on community resources to help clients set goals, access services, and build stability.
Pre-Release Coordinator
We’re seeking a compassionate, dedicated individual to support justice-involved clients preparing for successful community re-entry. This role involves facilitating pre-release workshops, providing personalized care coordination, mentoring clients, connecting them with essential community resources, and supporting clients in overcoming barriers to ensure long-term stability
