Pre-Release Services
Transition:
After release, we will work with individuals to make sure all basic needs are met and act as their advocate and help navigate them through any situation. Together, we develop a plan for successful re-entry back into the community and support them in reaching their goals.
Orientation:
All new individuals start by attending an orientation where they will learn about the programs and services available at PACE. Orientation occurs every other week. Upon completion, individuals will meet with staff individually to determine appropriate program placement.
Coordinated Case Management:
After completing Orientation, individuals will be assigned a case manager and coach to support them throughout their participation in PACE programs and services. With staff, they will work together to address any and all barriers to successful re-entry into the community.
